Tip: How to add a provider

The “Add Provider” screen allows you to search for an existing provider or add a new provider to the database.

Fill out as much detail as you have for your provider if not found in the database.

Tip: Authorizing Record Collection

Be aware of the checkbox, “Link this Provider to a Member to enable record collection.”

This allows you to link a provider with a member in the Zweena account to enable record collection. After adding the provider, with the checkbox checked, you are asked to enable the collection of that provider’s records on behalf of the Zweena member. Select the Zweena member who is a patient of the provider and click “Authorize Record Collection.”

Please note: You will need to fill out a Authorization to Release Medical Records form with each new provider, per law requirements.

How to Upload Your Medical Records

Zweena gives you the freedom to upload your own medical records!

There are ten quick-and-easy steps to this process:

  1. Step One: You need to have a copy of your records in the first place! These need to be saved as a .PDF on your local computer.
  2. Step Two: Sign into your Zweena Health account.
  3. Step Three: Click on the “Zweena Health Records” tab.
  4. Step Four: Then click “Upload Documents” on the left (see below)

  6. Step Five: Select the individual you would like to upload medical documents to.
  7. Step Six: Find the scanned record by clicking “Choose File” to select the .PDF, .JPG, .PNG or .TIFF of the medical record on your local computer.
  8. Step Seven: Select both the appointment date and the visit type (organizational tip!).
  9. Step Eight: This section allows you to sync the uploaded medical record with an existing provider in the account or to create a new provider.
  10. Step Nine: You are given the option to “digitize” the data. Digitizing medical records transforms them from scanned paper record to data in structured fields, which gives users access to dashboard functionality.
  11. Step Ten: Click “Upload Document” and your document will be uploaded!

    This is what an uploaded document looks like. You are able to view the scanned document or click “digitize” to transform this scanned paper record into structured data that becomes data for the Zweena dashboard.

How to Have Zweena Collect Your Records

Want Zweena to collect your records?

No problem! Zweena Health is able to collect medical documents on behalf of its members.

  1. A signed Zweena Health HIPAA Authorization Form to Release Health Information. Download the sheet by clicking the picture below:

  2. Providers added to an account. The Zweena clinical coordinator team needs to know who to contact and on whom’s behalf.

How to Share Your Medical Records

Zweena offers the unique benefit of being able to share medical records with providers, family members and more. You can only share a medical record if you have been granted custodial access.

To view the sharing area within your Zweena account, log in and select the “Shared Records” tab.

The left-hand side allows you to select from the different categories of shared records, as listed above.

Select the level of sharing on the left, then select the individual’s health record to share, with whom it’s been shared with, and whether you would like to have the record sent via email or printed.

See the status of all of your shared records at the bottom of this page, including clicking “edit” to change the level of access or revoke access completely.

Remember, you control what the person you share your PHR with can do with the information. Some information stored in the health records you manage may be highly sensitive. Before you grant access to a record, consider carefully which people should be allowed to see the information.

How to Create an Appointment

Creating an appointment gives the Zweena team a heads-up that we will have medical information to collect.

To create an appointment follow these three steps:

  1. Click “Add New Appointment” underneath a member’s name in the “My Community” tab.
  2. Select the upcoming date, Zweena member whose appointment it is, the provider and reason for the appointment.

How to Add a Family Member

Your Zweena Health account allows you to add family members. This is useful to manage all of your family’s medical records in one account!

To add a family member, from the “My Community” tab, click “+Add members”.

The next screen takes you to Microsoft HealthVault to create a new family member.

Click “+Add a person” underneath your information, fill out the corresponding information and click “Create record.”

After clicking “Allow access” on the Microsoft HealthVault screen, you are taken back to your “My Community” Zweena tab and your new family member is now listed in your Zweena account.
